Impact: change existing irq_chip API
Not much point with gentle transition here: the struct irq_chip's
setaffinity method signature needs to change.
Fortunately, not widely used code, but hits a few architectures.
Note: In irq_select_affinity() I save a temporary in by mangling
irq_desc[irq].affinity directly. Ingo, does this break anything?

After adding the RTC platform device to malta-platform.c malta-mtd.c should
get unified with the rest of the platform device code.

With the conversion of MIPS to RTC_LIB the old RTC driver CONFIG_RTC became
unselectable. Fix by setting up a platform device. Also enable
RTC_CLASS so system time gets set from RTC on kernel initialization.

On linux-queue, malta doesn't build after the include file relocation.
This should fix it.
There some occurrences of 'asm-mips' in the comments of quite a few
files, but this is the only place I found it in any code.

Rework of SMTC support to make it work with the new clock event system,
allowing "tickless" operation, and to make it compatible with the use of
the "wait_irqoff" idle loop. The new clocking scheme means that the
previously optional IPI instant replay mechanism is now required, and has
been made more robust.

This patch explicitly removes the kgdb implementation, for mips which
is intended to be followed by a patch that adds a kgdb implementation
for MIPS that makes use of the kgdb core in the kernel.
