fix the following 'make headers_check' warnings:
usr/include/asm/ptrace-abi.h:86: include of <linux/types.h> is preferred over <asm/types.h>
usr/include/asm/ptrace-abi.h:93: found __[us]{8,16,32,64} type without #include <linux/types.h>

Change header guards named "ASM_X86__*" to "_ASM_X86_*" since:
a. the double underscore is ugly and pointless.
b. no leading underscore violates namespace constraints.
