When the EEPROM contains weird values for the power levels we have to
fix the interpolation process.

documentation on rfbufer.h
* Merge all rf regs functions to one
* Sync with legacy HAL and Sam's HAL
* Set gain_F settings so that gain_F optimization engine works
on RF5111/RF5112 (note that both HALs only use step 0 for RF5111
and they don't use gain_F optimization for this chip, code is
there but is never used)

* Update and cleanup rf gain optimization code
* Add comments and refferences to docs and use sane function names
* Use only step index on ath5k_gain, no need to have a pointer to
the current step since we can determine te step from it's index,
this also allows us to put all other structs on rfgain.h and cleanup
ath5k.h a little
* No need for ah_rfgain variable, we use ah_gain.g_state for everything
* Tested on RF2112B chip but gain_F calibration is not yet done
(we will finish this on the next patch where we'll rewrite rf-buffer
handling)
* Use initial rf gain settings for 2316 and 2317 SoCs introduced on a previous patch
It seems big but it's mostly cleanup, very few functional changes have been made on phy.c

* Clean up initial rf buffer settings (new file rfbufer.h) and introduce a
new way to access specific rf registers (will use it later)
* Clean up initial rf gain settings by moving them on a new file (rfgain.h)
so we can later work on gain optimization functions
* Update initial rf buffer settings and initial rf gain settings from HALs.
This breaks things for now because our current dumps come from pre-configured
rf buffer (regdumps already had the needed values set from binary HAL).

Noise floor calibration occasionally fails on Atheros hardware.
This is not fatal and can happen if there's simply too much
noise on the air. Ignoring the calibration error is the right
thing to do here, because when the error is ignored, the hardware
will still work, whereas if the error causes the driver to bail out
of a bigger configuration function and does not configure the tx
queues or the IMR (as is the case in reset.c), the hw no longer
works properly until the next reset.

A few changes to reduce checkpatch.pl errors in the ath5k driver. For
the most part, I only fixed cosmetic things, and left the actual 'code
flow' untouched (hopefully)!
Diff is against wireless-testing HEAD.

on ath_info) so it's better this way.
* Rename following functions to maintain naming scheme:
ah_setup_xtx_desc -> ah_setup_mrr_tx_desc
(Because xtx doesn't say much, it's actually
a multi-rate-retry tx descriptor)
ath5k_hw_put_tx/rx_buf - > ath5k_hw_set_tx/rxdp
ath5k_hw_get_tx/rx_buf -> ath5k_hw_get_tx/rxdp
(We don't put any "buf" we set descriptor pointers on hw)
ath5k_hw_tx_start -> ath5k_hw_start_tx_dma
ath5k_hw_start_rx -> ath5k_hw_start_rx_dma
ath5k_hw_stop_pcu_recv -> ath5k_hw_stop_rx_pcu
(It's easier this way to identify them, we also
have ath5k_hw_start_rx_pcu which completes the
set)
ath5k_hw_set_intr -> ath5k_hw_set_imr
(As in get_isr we set imr here, not "intr")
* Move ath5k_hw_setup_rx_desc on ah->ah_setup_rx_desc so we can
include support for different rx descriptors in the future
* Further cleanups so that checkpatch doesn't complain
(only some > 80 col warnings for eeprom.h and reg.h as usual
due to comments)
Tested on 5211 and 5213 cards and works ok.

were only enabling calibration during reset). If we don't we commit
the same results each time calibration routine is called.

* Since RF2413 it seems that RF_BUFFER settings are different (notice
that the last part -"bank 7" or whatever- is smaller than in 5111/5112).
So until we know what's going on we assume there is no gain optimization
stuff in post-5112 chips.

Cleanup after API changes patch (checkpatch.pl stuff) and on
ath5k_hw_channel() make use of the existing ath5k_channel_ok()
instead of re-implementing the checks again. This was necessary
to make the code cleaner and fit the 80-chars wide limit so
sending it within the same patch.
Finally make a note that we *may* eventually move cap_range stuff
to struct wiphy (band frequency range capabilities). This
information can later be exported to userspace, for example,
and giving it access to mac80211 and drivers in general can come
in handy.

Also i figured out a way to process rate vallue found
on status descriptors, it's still buggy but we are getting
closer (i think it improved stability a little).
