Since the ehca device driver does not support dynamic memory add and
remove operations, the driver must explicitly reject such requests in
order to prevent unpredictable behaviors related to existing memory
regions that cover all of memory being used by InfiniBand protocols in
the kernel.
The solution (for now at least) is to add a memory notifier to the
ehca device driver and if a request for dynamic memory add or remove
comes in, ehca will always reject it. The user can add or remove
memory by hot-removing the ehca adapter, performing the memory
operation, and then hot-adding the ehca adapter back.

Because ehca adapters can differ in the maximum number of QPs and CQs
we have to save the maximum number of these ressources per adapter and
not globally per ehca driver. This fix introduces 2 new members to the
shca structure to store the maximum value for QPs and CQs per adapter.
The module parameters are now used as initial values for those
variables. If a user selects an invalid number of CQs or QPs we don't
print an error any longer, instead we will inform the user with a
warning and set the values to the respective maximum supported by the
HW.

moving the code that computes the features into generic function which is
now called through non-child and child device creation.

The Mellanox ConnectX can operate as an InfiniBand adapter, as an
Ethernet NIC, or as a Fibre Channel (FC) HBA. The kernel has a
low-level driver, mlx4_core, which handles multiplexing access to the
device, and there is also already an InfiniBad driver, mlx4_ib.
This patch adds a new driver, mlx4_en, which implements a standard
Ethernet NIC driver.

Multi-protocol adapters support different port types. Each consumer
of mlx4_core queries for supported port types; in particular mlx4_ib
can no longer assume that all physical ports belong to it. Port type
is configured through a sysfs interface. When the type of a port is
changed, all mlx4 interfaces are unregistered, and then registered
again with the new port types.

For ethernet support, we need to reserve QPs for the ethernet and
fibre channel driver. The QPs are reserved at the end of the QP
table. (This way we assure that they are aligned to their size)
We need to consider these reserved ranges in bitmap creation, so we
extend the mlx4 bitmap utility functions to allow reserved ranges at
both the bottom and the top of the range.

Use krealloc() instead of kmalloc() followed by memcpy() when resizing
the MAD module's snoop table.
Also put parentheses around the new table size to avoid calculating
the wrong size to allocate, which fixes a bug pointed out by Haven
Hash <haven.hash@isilon.com>.

ipoib_ib_dev_stop() does del_timer_sync(&priv->poll_timer), but if a
P_key for an interface is not found, poll_timer is not initialized, so
this leads to a crash or hang. Fix this by moving where poll_timer is
initialized to ipoib_ib_dev_init(), which is always called.

To allow allocating an aligned range of consecutive QP numbers, add an
interface to reserve an aligned range of QP numbers and have the QP
allocation function always take a QP number.
This will be used for RSS support in the mlx4_en Ethernet driver and
also potentially by IPoIB RSS support.

In case of error, the function ucma_alloc_multicast() returns a NULL
pointer, but never returns an ERR pointer. So after a call to this
function, an IS_ERR test should be replaced by a NULL test.

We already did that a long time ago for pnp_system_init, but
pnpacpi_init and pnpbios_init remained as subsys_initcalls, and get
linked into the kernel before the arch-specific routines that finalize
the PCI resources (pci_subsys_init).
This means that the PnP routines would either register their resources
before the PCI layer could, or would be unable to check whether a PCI
resource had already been registered. Both are problematic.
I wanted to do this before 2.6.27, but every time we change something
like this, something breaks. That said, _every_ single time we trust
some firmware (like PnP tables) more than we trust the hardware itself
(like PCI probing), the problems have been worse.

Don't wait between submitting crypt requests for a bio unless
we are short of memory.
There are two situations when we must split an encrypted bio:
1) there are no free pages;
2) the new bio would violate underlying device restrictions
(e.g. max hw segments).
In case (2) we do not need to wait.
Add output variable to crypt_alloc_buffer() to distinguish between
these cases.

start of crypt_convert().
Introduce crypt_finished to indicate whether or not the encryption
is finished, for use in a later patch.

The pending reference count must be incremented *before* the async work is
queued to another thread, not after. Otherwise there's a race if the
work completes and decrements the reference count before it gets incremented.

dm-raid1 is setting the 'DM_KCOPYD_IGNORE_ERROR' flag unconditionally
when assigning kcopyd work. kcopyd is responsible for copying an
assigned section of disk to one or more other disks. The
'DM_KCOPYD_IGNORE_ERROR' flag affects kcopyd in the following way:
When not set:
kcopyd will immediately stop the copy operation when an error is
encountered.
When set:
kcopyd will try to proceed regardless of errors and try to continue
copying any remaining amount.
Since dm-raid1 tracks regions of the address space that are (or
are not) in sync and it now has the ability to handle these
errors, we can safely enable this optimization. This optimization
is conditional on whether mirror error handling has been enabled.

This patch allows path errors from the multipath ctr function to
propagate up to userspace as errno values from the ioctl() call.
This is in response to

https://bugzilla.redhat.com/show_bug.cgi?id=444421
The patch only lets through the errors that it needs to in order to
get the path errors from parse_path().

Enable driver checking of the DMI product name (when enabled) on
an Abit AT8 32X, instead of falling back to a manual probe. This
eliminates false negatives and eventually will help avoid
unnecessary bus probes on unsupported mainboards.

The table for the Abit AT8 32X was incorrectly missing an entry
for the sixth ("AUX3") fan. Add this entry, exporting the fan
reading to userspace.

On the Shuttle SN68PT, FAN_CTL2 is apparently not connected to a fan,
but to something else. One user has reported instant system power-off
when changing the PWM2 duty cycle, so we disable it.
I use the board name string as the trigger in case the same board is
ever used in other systems.

Creates a name file in the sysfs directory, that

Since all bio_split calls refer the same single bio_split_pool, the bio_split
function can use bio_split_pool directly instead of the mempool_t parameter;
then the mempool_t parameter can be removed from bio_split param list, and
bio_split_pool is only referred in fs/bio.c file, can be marked static.

This patch converts gdrom to use __blk_end_request() directly
so that end_{queued|dequeued}_request() can be removed.
gd.transfer is '1' in error cases and '0' in non-error cases,
so gdrom hasn't been propagating any error code to the block layer.
We can just convert error cases to '-EIO'.
