Delete mark_inode_dirty() in nilfs_commit_chunk(), for callers of
nilfs_commit_chunk() will call equivalent mark_inode_dirty()
after calling nilfs_commit_chunk().

change return type of nilfs_commit_chunk() as void from int,
for nilfs_set_file_dirty() usually does not return error.
This is an intermediate patch to reduce mark_inode_dirty() in
nilfs_commit_chunk().

* Remove smp_lock.h from files which don't need it (including some headers!)
* Add smp_lock.h to files which do need it
* Make smp_lock.h include conditional in hardirq.h
It's needed only for one kernel_locked() usage which is under CONFIG_PREEMPT
This will make hardirq.h inclusion cheaper for every PREEMPT=n config
(which includes allmodconfig/allyesconfig, BTW)

Pekka Enberg suggested converting ->ioctl operations to use
->unlocked_ioctl to avoid BKL.
The conversion was verified to be safe, so I will take it on this
occasion.
