Address a long standing issue of booting with an initrd on an i386 numa
system. Currently (and always) the numa kva area is mapped into low memory
by finding the end of low memory and moving that mark down (thus creating
space for the kva). The issue with this is that Grub loads initrds into
this similar space so when the kernel check the initrd it finds it outside
max_low_pfn and disables it (it thinks the initrd is not mapped into usable
memory) thus initrd enabled kernels can't boot i386 numa :(
My solution to the problem just converts the numa kva area to use the
bootmem allocator to save it's area (instead of moving the end of low
memory). Using bootmem allows the kva area to be mapped into more diverse
addresses (not just the end of low memory) and enables the kva area to be
mapped below the initrd if present.
I have tested this patch on numaq(no initrd) and summit(initrd) i386 numa
based systems.

The following series implements memory hot-add for ppc64 and i386. There are
x86_64 and ia64 implementations that will be submitted shortly as well,
through the normal maintainers.
This patch:
local_mapnr is unused, except for in an alpha header. Keep the alpha one,
kill the rest.

If CONFIG_NUMA isn't set, we use the define in <linux/mmzone.h> for
early_pfn_to_nid (which defines it to 0).
Because of this, the prototype needs to move inside the CONFIG_NUMA too, or
anal gcc's get really confused.

Provide a default implementation for early_pfn_to_nid returning node 0. Allow
architectures to override this with their own implementation out of
asm/mmzone.h.

This patch effectively eliminates direct use of pgdat->node_mem_map outside
of the DISCONTIG code. On a flat memory system, these fields aren't
currently used, neither are they on a sparsemem system.
There was also a node_mem_map(nid) macro on many architectures. Its use
along with the use of ->node_mem_map itself was not consistent. It has
been removed in favor of two new, more explicit, arch-independent macros:
pgdat_page_nr(pgdat, pagenr)
nid_page_nr(nid, pagenr)
I called them "pgdat" and "nid" because we overload the term "node" to mean
"NUMA node", "DISCONTIG node" or "pg_data_t" in very confusing ways. I
believe the newer names are much clearer.
These macros can be overridden in the sparsemem case with a theoretically
slower operation using node_start_pfn and pfn_to_page(), instead. We could
make this the only behavior if people want, but I don't want to change too
much at once. One thing at a time.
This patch removes more code than it adds.
Compile tested on alpha, alpha discontig, arm, arm-discontig, i386, i386
generic, NUMAQ, Summit, ppc64, ppc64 discontig, and x86_64. Full list

Boot tested on NUMAQ, x86 SMP and ppc64 power4/5 LPARs.

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.
Let it rip!
