This fixes a problem with gcc4 mis-compiling the stack unwind code under
-Os, which resulted in 'stuck' messages whenever an assembly routine was
encountered.
(The second hunk is trivial cleanup.)

This changes the dwarf2 unwinder to do a binary search for CIEs
instead of a linear work. The linker is unfortunately not
able to build a proper lookup table at link time, instead it creates
one at runtime as soon as the bootmem allocator is usable (so you'll continue
using the linear lookup for the first [hopefully] few calls).
The code should be ready to utilize a build-time created table once
a fixed linker becomes available.

Current gcc generates calls not jumps to noreturn functions. When that happens the
return address can point to the next function, which confuses the unwinder.
This patch works around it by marking asynchronous exception
frames in contrast normal call frames in the unwind information. Then teach
the unwinder to decode this.
For normal call frames the unwinder now subtracts one from the address which avoids
this problem. The standard libgcc unwinder uses the same trick.
It doesn't include adjustment of the printed address (i.e. for the original
example, it'd still be kernel_math_error+0 that gets displayed, but the
unwinder wouldn't get confused anymore.
This only works with binutils 2.6.17+ and some versions of H.J.Lu's 2.6.16
unfortunately because earlier binutils don't support .cfi_signal_frame

If no unwinding is possible at all for a certain exception instance,
fall back to the old style call trace instead of not showing any trace
at all.
Also, allow setting the stack trace mode at the command line.

These are the generic bits needed to enable reliable stack traces based
on Dwarf2-like (.eh_frame) unwind information. Subsequent patches will
enable x86-64 and i386 to make use of this.
Thanks to Andi Kleen and Ingo Molnar, who pointed out several possibilities
for improvement.
